  • 211 Sonoma County

    211 Sonoma is a comprehensive referral platform that connects Sonoma County callers, website-users and texters each year with information about health and human services available to them.

    Partnership Health Plan has a resource directory where you can find a variety of resources.

Diversity, Equity, and Belonging + Community Engagement

Sonoma Connect | Sonoma Unidos is committed to equity and doing the internal and external work needed to dismantle systems that currently create barriers for black, indigenous, people of color communities to access equitable support and care. The following are tools designed to help our network practice equity.

  • Diversity, Equity, and Belonging Glossary

    Sonoma Connect | Sonoma Unidos partners developed this Diversity, Equity, and Belonging Glossary to inform and provide clarity on the work that is taking place in our action teams.

  • Sonoma Connect's Stipend Procedure

    Sonoma Connect | Sonoma Unidos is putting into place this stipend procedure to support and incentivize community members that would not otherwise be able to participate. Stipends will be made available if SC | SU has funds available that include CE stipends as an allowable expense

Training and Workforce Development

Sonoma Connect | Sonoma Unidos community partners provide training opportunities for workforce development and capacity/knowledge building.

  • Center for Wellbeing

    The Center for Wellbeing provides professional development classes and training to Community Health Workers/Promotores to help curb the effects of diabetes, arthritis, cardiovascular disease, obesity, and more.

  • Community Health Worker Monthly Meetings

    The Center for Wellbeing hosts monthly Community Health Worker Network Meeting: meetings for Community Health Workers/Promotores de Salud to learn about community resources, important topics, network with others, and receive support

  • Hanna Institute

    Hanna Institute equips and empowers individuals and organizations with the trauma-informed tools they need to help people in their community. From comprehensive trauma-informed care (TIC) training and certifications for individuals and organizations to large-scale program partnerships, the Institute works alongside youth and the community to break the cycle of trauma and create a healthier and more hopeful future for all. 

Screening and ACEs Resources

Sonoma Connect | Sonoma Unidos has provided Health Centers and CBOs assistance and support to expand ACEs screening in Sonoma County. To learn more about how to become an attested screener for ACEs or to access support to expand your organization’s trauma-informed capacity check out the resources below:

  • Santa Rosa Community Health ACEs Screening Clinical Workflow

    This resource was developed by Santa Rosa Community Health and was shared in Sonoma Connect | Sonoma Unidos’ ACEs Screening Implementation Action Team. It shows the procedure they follow for screening their patients for ACEs. This was shared with the intention of helping other entities establish their own ACEs screening workflow.

  • Sonoma County PACEs Connection

    Sonoma County PACEs Connection is a local forum for individuals or organizations to find information, resources, and support for ACEs.

  • ACEs Aware

    The ACEs Aware initiative is a first-in-the nation effort to screen patients for Adverse Childhood Experiences (ACEs) to help improve and save lives.

    ACEs Aware strives to create a better world for our children, families, and communities by working together across sectors to prevent and address the impact of ACEs and toxic stress.

  • My Number Story

    My Number Story provides educational awareness about ACEs, the impact that it has on a person’s health, and provides resources and information to curb the negative effects of ACEs.
