Action Team: Resource Connection Network

This team works to build a sustainable medical and social system integration tool using technology to connect existing platforms across sectors. This technology infrastructure is meant to create a more cohesive and operationally manageable ecosystem to support service care delivery and ultimately our community. The platform will support comprehensive high-quality resources that support communication, referrals, data sharing, and shared case management between organizations.

Meet Our Resource Connection Network Action Team Co-chairs

Jade Weymouth, Executive Director at La Familia Sana

Growing up multi-racial as 2nd generation Chinese- American and 3rd generation Mexican-American, Jade knew that she was her ancestors dreams. Her family was very involved in the United Farm Worker (UFW) Movement in the Gilroy, Salinas and Watsonville area. Her great grandfather, Antionio Del Buono, was a labor organizer and farmworker and her grandmother, Cecilia Weymouth, was an educator and organizer. At a young age, service and volunteerism were instilled in her. Now she hopes to continue this legacy with her own daughter in her own community.

Jade (she/her) has a Bachelor's degree in Women's Studies from UC Santa Cruz and a near-complete Master's degree in Ethnic Studies. After founding a non-profit called fundakid, which ensured all youth access to high-quality programs and educational experiences, she worked with Boys & Girls Clubs of Sonoma-Marin, serving children and their families in Northern Sonoma County. There, Jade led a 20-person team overseeing 500+ youth, three school districts, and several stakeholders and community members. In tandem, she earned her Master's in Organization Development from Sonoma State. As a founding member of La Familia Sana, Jade came on board as the Executive Director of La Familia Sana in 2021. 

There, she works to implement the organization's core values of grassroots advocacy, empowerment, and equity throughout the organization and northern Sonoma County community.

Cynthia King, Executive Director at Community Action Partnership Sonoma

Cynthia has over 20 years of experience in the public and non-profit building programs and data systems that facilitate systems change. After completing UC Berkeley’s dual degree program with an MPH in Maternal and Child Health and an MSW in Medical Social Work she spent 9 years as an Data and Evaluation Analyst at Alameda County Social Services Agency, moving to Sonoma County Human Services for a year at Upstream Investments.

Since then she has spent time at social service and public health non-profits. She worked for Redwood Community Health Coalition (RCHC), an organization that supports Community Health Centers in 6 Bay Area/NorCal Counties. At RCHC she was the Senior Director of Programs, overseeing various programs in Population Health, Health Informatics, and Outreach and Enrollment. She and her team have been driving and/or supporting CIE work in four counties. Currently, Cynthia is the Executive Director for Frontline Workers Counseling Project (FWCP), an award-winning, Bay Area-based, grassroots charitable organization that connects frontline and essential workers with free high-quality psychotherapy and support groups offered by licensed professional psychotherapists. Cynthia is the chair of the Resource Connection Network Action team managing the technology selection and is also on the Community Leadership Board. She has also been participating in the CBAN Bay Area CIE meetings.
