Event Four
Resource Connection Network Action Team Meeting
Working to build a sustainable medical and social system integration tool using technology to connect existing platforms wherever possible. The platform will support comprehensive high quality resources that support, communication, referrals and shared case management between organizations.

Event Three
Community Accountability Action Team Meeting
Ensures that Sonoma Connect | Sonoma Unidos addresses racism and social inequities. This action team works to uplift community voices and support pathways to leadership for BIPOC communities in the county. The Community Accountability Action team supports building and fostering a system of authentic community engagement by connecting to existing efforts, and supporting local leaders.

Event Two
Neighborhood Resource Centers Action Team Meeting
Focuses on the people and places that make a resource connection system work: community health workers (CHW’s) and neighborhood resource centers (NRCs). This action team develops standards of practice to build workforce capacity for CHW’s and NRC’s. The NRC action team provide trauma-informed support and referrals and close service gaps.

Event One
ACE Screening Clinical Implementation Action Team Meeting
Considers and implements how to build on best practices for ACE screenings, education and responsive support where needed in Sonoma County. The current focus of this action team is on how health centers can partner with schools, community-based organizations, and government agencies to integrate medical and social health. Health centers, schools and community-based organizations currently attend.
